Dr. William Karnes offers additional mechanisms to request and schedule consultations and procedures for your patients:

            1) Complete and submit Online Service Request Form; or

            2) Download and complete a PDF Service Request Form (or use your own request form) and deliver to Dr. Karnes' office:

                - electronically (email button on the form)
                - by fax (805-618-1820) or;
                - mail or;
                - ask your patient to hand-carry to our office (9A-5P, M-Th).

Your patient will be asked to complete an Online Registration Form which they may complete and submit online, or complete with pen & paper by visiting our office.

               1) recent H&P and any other clinically relevant data, including labs and x-rays

            2) insurance information
